CATIE and Typical Antipsychotics

ثبت نشده
چکیده

Introduction Atypical antipsychotics have largely replaced typical antipsychotics in clinical practice due to their perceived superiority in efficacy and lower risk of neurological adverse effects, which is believed to result in improved compliance. In fact, many treatment algorithms recommend the use of atypical antipsychotics as the only first-line medication treatment option. However, the Clinical Antipsychotic Trials of Intervention Effectiveness (CATIE) has cast doubts over the superiority of atypical antipsychotics over the older, less expensive typical antipsychotics. More recently, an independent British study of similar design has also had similar findings to that of CATIE. This bulletin will review the results of CATIE and revisit the use of oral typical antipsychotics in clinical practice.

Antipsychotics and metabolics in the post-CATIE era.

Schizophrenia patients have high prevalence of cardiovascular (CV) disease risk factors and high CV mortality, with increasing concern over the contribution of antipsychotic medications to cardiometabolic risk. The design of the NIMH-sponsored Clinical Antipsychotic Trials of Intervention Effectiveness (CATIE) Schizophrenia Trial was driven by a need to understand the efficacy and safety differ...

Where Does Evidence from New Trials for Schizophrenia Fit with the Existing Evidence: A Case of the Emperor's New Clothes?

Advent of "atypical" antipsychotics has spawned new trials in the recent years and the number of such trial reports has been increasing exponentially. As clinicians we have been led to believe that "atypicals" are better than "typicals" despite the odd dissenting voice in academic and clinical circles. This has been largely ignored until the publication of two landmark, independent, pragmatic t...

Comparative effectiveness research for antipsychotic medications: how much is enough?

Second-generation antipsychotics have attracted practitioners' and policy-makers' attention, because of concerns over their health effects and costs. Comparative effectiveness data from the Clinical Antipsychotic Trials of Intervention Effectiveness (CATIE)-a high-profile National Institutes of Health (NIH)-funded study-have been used to argue for restricting coverage for these costly drugs. Bu...

All patients with a confirmed diagnosi...

Genotypic variation in the SV2C gene impacts response to atypical antipsychotics the CATIE Study

Pharmacogenetic (PGx) predictors of response would improve outcomes in antipsychotic treatment. Based on both biological rationale and prior evidence of an impact on Parkinson's disease, we conducted an association study for 106 SNPs in the synaptic vesicle protein 2C (SV2C) gene using genetic and treatment response data from the Clinical Trial of Antipsychotic Intervention Effectiveness (CATIE...
